On-The-Job Meal Agreement

     This is to verify that I am aware and agree that when the nature of the work prevents me as an employee from being relieved of all duty during my meal period, I will take an on-duty meal, and I will be paid for the time.

     I also understand that I have the right to withdraw this On-The-Job Meal Agreement at any time.

Waiver of Second Meal Period

     I am aware that when my workday is over 10 hours I am entitled to a second meal period. The law allows me to waive the second meal period if I have had my first meal period and if the work can be completed in 12 hours.

     The foregoing being the case, I wish to waive my second meal period.
